Habilitation Service
Welcome to Amazing Grace Foundations, our Habilitation Training Specialist (HTS) service provides individualized, hands-on support for individuals with intellectual and developmental disabilities in their home or community setting.

We focus on building independence through daily living skills, such as personal care, health and safety awareness, and community involvement- helping each individual gain confidence and improve their overall quality of life.
HTS services are available to individuals who qualify through the waiver program for developmental disability programs and are delivered by trained caregivers under the supervision of a provider agency.

Our dedicated team assists with daily living activities, such as personal care, household tasks, and medication management.
We focus on building life skills, encouraging independence and enhancing overall quality of life in a supporting and engaging environment.

Transportation Services
Safe and reliable transportation services are provided for medical appointments, employment, community outings, recreational activities, and personal errands. All drivers are properly licensed and insured to ensure the safety and well-being of the individuals served.
Individuals are encouraged to actively participate in planning and coordinating their transportation needs to promote independence, choice, and community involvement.
